ANNUAL ENROLLMENT:
Begins November 1st, 2011


You may be eligible to join this plan if:

  1. You live in the State of Louisiana; and

  2. You are a retired State employee eligible to enroll in Original Medicare because of age or disability, currently enrolled in Original Medicare, or you are currently enrolled in an OGB-sponsored Medicare Advantage plan.

Begins October 3, 2011 to November 4, 2011 for a January 1st, 2012 effective date.

You may be eligible to join this plan if:

  1. Members must reside in Regions 6, 7, 8 and 9.

  2. You are an eligible active State employee OR State retiree AND are not currently enrolled in one of OGB's Medicare Avdantage Plans.

  1. You wish to obtain ALL of your medical care from your choice of Medical Home Primary Care Physicians in Region 9.

  2. You are an eligible active State employee OR State retiree AND are not currently enrolled in one of OGB's Medicare Avdantage Plans.
